BIRTHDAYS
90th birthday letters can be requested from:

50th (Golden) anniversary letters can be requested from:

100th, 105th and 120th birthday letters can be requested from:

60th, 65th and 70th (Diamond) birthday letters can be requested from:

How to request a letter
STEP 1
Organise a copy of a supporting document to verify the applicant’s date of birth or marriage. These documents include:
- Birth or marriage certificate (photocopies are acceptable)
- Passport or citizenship papers (photocopies are acceptable)
- OR
- Statutory declaration to confirm date of birth, or place and date of marriage. The statutory declaration should be signed by a member of the family and witnessed by a Justice of the Peace or Commissioner for Declarations. STEP 2

STEP 3
Submit the completed form and supporting materials to our office at least 6 weeks prior to the anniversary so that the letters can be delivered in time.
